The goal for the CHLCC program is to (1) reduce type 2 diabetic incidence rates by 60% (2) reduce obesity by 28%, and (3) improve unhealthy eating habits by 90% among African American women, ages 45-55 years, in Clayton county Georgia, within the next three years (see figure 1; Breny, Fagen, & Roe, 2017, p. 147; Petrie, 2018). A pilot of the program will be implemented at Southern Regional Medical Center (Riverdale), in Clayton county, GA. Partnerships will be developed and form with social workers and physicians to enroll prediabetic non-Hispanic Black female participants.
